GDPR (implemented by DPA2018 in the UK) brought in new legal protection for personal information from May 2018. This tells you what personal information I hold and why, and what your rights are. Once you have read it please complete and sign the declaration/statement of consent at the bottom.
Lawful Basis for holding and using Client Information
As a full member of the Association of Reflexologists, I abide by the AoR Code of Practice and Ethics. The lawful basis under which I hold and use your information is my legitimate interest’s i.e.my requirement to retain the information in order to provide you with the best possible treatment options and advice
As I hold special category data (i.e. health related information), the Additional Condition under which I hold and use this information is: for me to fulfil my role as a health care practitioner bound under the AoR Confidentiality as defined in the AoR Code of Practice and Ethics.
What information I hold and what I do with it
In order to give professional reflexology treatments, I will need to ask for and keep information about your health. I will only use this for informing reflexology treatments and any advice I give as a result of your treatment. The information to be held is:
• Your contact details
• Medical history and other health-related information (which I will take from you at first consultation)
• Treatment details and related notes (which I will take after each consultation)
I will NOT share your information with anyone else (other than within my own practice, or as required for legal process such as NHS Track and Trace.) without explaining why it is necessary, and getting your explicit consent.
It may be helpful for your treatment for me to share your information with: with other Reflexologist and podiatrists.

Protecting Your Personal Data
I am committed to ensuring that your personal data is secure. In order to prevent unauthorised access or disclosure, I have put in place appropriate technical, physical and managerial procedures to safeguard and secure the information we collect from you.
I will contact you using the contact preferences you give me in relation to:
• Appointment times
• Reflexology information or information related to your health
• Special offers and promotions (you may unsubscribe from this at any time)
Your Rights
GDPR gives you the following rights:
• The right to be informed:
To know how your information will be held and used (this notice).
• The right of access:
To see your therapist’s records of your personal information, so you know what is held about you and can verify it.
• The right to rectification:
To tell your therapist to make changes to your personal information if it is incorrect or incomplete.
• The right to erasure (also called “the right to be forgotten”):
For you to request your therapist to erase any information they hold about you
• The right to restrict processing of personal data:.
You have the right to request limits on how your therapist uses your personal information
• The right to data portability: under certain circumstances you can request a copy of personal information held electronically so you can reuse it in other systems.

To be able to tell your therapist you don’t want them to use certain parts of your information, or only to use it for certain purposes.

• The right to lodge a complaint with the Information Commissioner’s Office:
To be able to complain to the ICO if you feel your details are not correct, if they are not being used in a way that you have given permission for, or if they are being stored when they don’t have to be.

How Long I Retain Your Information for
I will keep your information for the following periods 7 years after last treatment.
THERAPIST’S RIGHTS
Please note:
• if you don’t agree to your therapist keeping records of information about you and your treatments, or if you don’t allow them to use the information in the way they need to for treatments, the therapist may not be able to treat you
• Your therapist has to keep your records of treatment for a certain period as described above, which may mean that even if you ask them to erase any details about you, they might have to keep these details until after that period has passed
• Your therapist can move their records between their computers and IT systems, as long as your details are protected from being seen by others without your permission.
I have seen this document and understand that you will hold and use my personal information, using it in order to provide me with the best possible treatment options and advice in line with the statements above.

1. About This Document
This document sets out the complaints procedure for Reflexology – Bizzy Feet Reflexology (referred to as “we”, “us” and “our” throughout this document). 3. Your Rights Under UK GDPR
You have the following rights in relation to the personal data we hold about you:

4. How to Make a Complaint

• The approximate date(s) when the issue occurred
• Any relevant reference numbers, correspondence or documents
5. Our Complaints Process
Once we receive your complaint, we will follow the five steps below. We are committed to handling all complaints promptly, fairly and confidentially.
1. Acknowledgement — within 30 days
We will acknowledge your complaint in writing within 30 days of receiving it, confirming that we have recorded it and will be investigating.
2. Requesting Further Information
If we need any additional details to fully investigate your complaint, we will contact you as soon as possible and explain what we need and why.
3. Investigation & Review
We will carry out a thorough and impartial review of your complaint. We will agree a realistic timescale with you once we have all necessary information, and we will keep you updated if there are any delays.
4. Decision & Outcome
We will communicate the outcome of our investigation to you clearly and in writing within one calendar month of receiving all the information needed (this may be extended by up to two further months for complex complaints — we will notify you if this is the case).
5. Closure or Escalation
If you are satisfied with the outcome, we will close your complaint. If you remain dissatisfied, you have the right to refer your complaint to the ICO free of charge (see Section 6 below).
6. Escalating Your Complaint to the ICO
If you remain dissatisfied with our response, you have the right to lodge a complaint with the Information Commissioner’s Office (ICO) — the UK’s independent supervisory authority for data protection. This service is free of charge.

• We may share this information with the Associate of Reflexologist’s advice team if I have anything relating to what I find or questions relating to your medical history.

Protecting your Personal Data - How we store your personal information
I am committed to ensuring that your personal data is secure. To prevent unauthorised access or disclosure, I have put in place appropriate technical, physical and managerial procedures to safeguard and secure the information we collect from you. I will contact you using the contact preferences you have given me.
We keep all your records, Name, address, email, phone numbers, medical history and treatment forms for 7 years. We will then dispose your information by shredding.
Your data protection rights
Under data protection law, you have rights including:
Your right of access - You have the right to ask us for copies of your personal information.
Your right to rectification - You have the right to ask us to rectify personal information you think is inaccurate. You also have the right to ask us to complete information you think is incomplete.

You are not required to pay any charge for exercising your rights. If you make a request, we have one month to respond to you.
